  2. Identification of the removed material and its former location

  3. A statement under penalty of perjury that the removal was a mistake or misidentification

  4. Your name, address, phone number, and email

  5. A statement consenting to the jurisdiction of U.S. federal courts and accepting service of process

Upon receipt of a valid counter-notification, we may restore the content unless the original complainant files a court action.

5) Misrepresentation
